Here is my drawing with what I hope is the correct way to do the
cocoon as I understand it.

1. Fold lower left corner b to b at top of picture.It will make a
triangle from a to b to e.
2. Seam from b to a for sleeve. There will be an opening for the hand
made by the distance from a to b. It will have some 'puffiness' made by
the increase/decrease at the beginning and end of pattern.
3. Fold lower corner c up to c at top of picture. It will make a
triangle from c to d to f. Seam sleeve from c to d. the result will look
something like the next picture.

>From b to c is the neck opening, from e to f is the hip/leg area. The 10
inch opening refers to the 5 inches left of center for the a-b-e
triangle fold, and 5 inches right of center for the c-d-f triangle
fold.The edge e-f will drape into a curve.
Does this help?
Also for whomever asked about the edging, the original pattern called
for a crab stitch (REVERSE single crochet) which simply put means single
crochet from left to right (backwards). It 'feels' awkwards at first,
but looks very nice. I have used it on the top of Scandinavian mittens
(which have no ribbing) and it is a very nice edge.
